Tasmania Birth Registration
Birth registration is compulsory and parents need to register their child's birth within 60 days of birth. A birth certificate can be obtained after the birth has been registered and both processes can be done at the same time.
Birth registeration and certificates are handled by the Tasmania Registry of Births, Deaths and Marriages. Visit the site for further information and contacts.
To Register:
Fill out and lodge a birth registration form and a notice of birth form with the Registry of Births, Deaths and Marriages. The hospital administration will give the documents to you, or if you have a homebirth you can obtain a registration form from the Registry.
When signing, parents can witness each other's signatures.
There is no fee required for registering a birth.
Information Needed To Register:
The date and place of birth, chosen names, surname and weight of the baby.
Baby’s date and place of birth, surname, first name/s and weight.
Mother’s surname, maiden name, given names, date and place of birth, usual residence, usual occupation, year of arrival in Australia if born overseas, aboriginality, and if married to the father, the date and place of marriage.
If the father’s details are to be recorded in the register then the same details will be required as that of the mother.
Unassisted Births:
The Notice of Birth form needs to be filled out by your husband, (or yourself if you are single), and a statement attached explaining why he has filled it out and not a hospital doctor or a midwife. The Birth Registration statement is for the parents to fill out.
To Obtain a Birth Certificate:
Fill out the birth certificate application. You can usually get them at the same time/place as the birth registration statement, plus this application is also available online in pdf format. You can get it here: Application for full certificate or extract
Decorative birth certificates are also available, and can be purchased by ticking the appropriate box on the application form. There appears to be only one style - you can view it here.
There are fees required for obtaining birth certificates. A full certificate or extract as of January 2005 is $31.92. Decorative certificates are $49.02 and this includes a standard full birth certificate copy as well.
Identification is also required in obtaining a birth certificate. You have to provide photocopies of identification documents. Only one form of identification is necessary if that identification is a drivers license, a defense force or police service card, or a passport.
If you do not have one of those, you need to provide two forms of id from the following; medicare card, credit card, educational report less than 12 months old, student ID card, or an entitlement card issued by the government.
